Children’s Discovery Museum of San Jose is hosting a vaccine clinic. Get vaccinated at the museum on February 13, 9:30am – 4:30pm, and receive a free museum vaxpass.
The “vaxpass” is valid for one museum visit for up to eight people the day of the clinic or through May 31, 2022 (one pass per vaccinated family).
Here’s what you need to know.
Walk-ups only on a first-come, first-served basis. Appointments are not necessary and cannot be made.
Children must be accompanied by a responsible caregiver over the age of 12. The responsible caregiver must stay with the minor for the duration of their time at the clinic (registration, vaccination, and observation).
Consent: vaccination documents need to be completed and signed by parent or legal guardian
a. This is where the actual parent or legal guardian comes in
b. Can be done prior, at home, does not have to be at the clinicAccompanying: physically at clinic, this can be a responsible caregiver 12 and older (sibling, family member, friend, etc)
c. Accompanying person only needs to be 12 or older
d. Does not have to be actual parent or legal guardian
- The vaccinations are free and no insurance is required.
- First and second doses are available for all eligible people, starting at age 5 and up.
- Pfizer Booster for people 12 and up (5 months after 2nd Pfizer dose); Moderna Booster for people 18 and up (6 months after 2nd Moderna dose).
- Proof of immigration status is not required. Staff will be onsite to support language needs and disability access. Call 711 for Hearing and Speech Relay Service.
